We begin the process of complaint resolution within 3 business days of receipt of this form. We strive to resolve complaints within 30 days of submission, though some policies and procedures may require an extension of that time frame.
Hitting Submit will save your progress. You then have the option to send it back to yourself to continue working, or submit it to the Provost's office.

Once the complaint is completely resolved, hit Submit to forward it to the Provost's office.
If the complaint is fully resolved, no action is needed. If additional work is needed, please Send Back to Previous Step to allow the previous person to continue working on it.
